I, the parent/guardian, or student listed above, do hereby agree to release all liability and claims against Evolve Martial Arts & Fitness and any instructor and/or party involved due to injuries, accidents, negligence, or any other circumstances arising from participation in all events such as parades, performances, clinics, demonstrations, tournaments, etc.
I sincerely pledge to obey all rules and regulations that are set up for the purpose of keeping order and for the protection of the students from injury. I recognize that a risk is involved in this art that requires my adherence to the rules and regulations and to the instructor(s) guidance and discipline. Because of the strenuous nature of Tae Kwon Do and combative activities, I understand that no one can positively assure that injury will not result. I hereby certify that all statements contained herein are true, and I have read the registration and full understand the contents of it.
Evolve Martial Arts & Fitness has my permission to use my or my child's photography publicly to promote Evolve Tae Kwon Do. I understand that the images may be used in print publications, online, presentations, websites, and social media. I understand that no fee should become payable to me for any reason for such use.

Assumption of risk / waiver of liability / indemnification agreement
In consideration of being allowed to participate on behalf of Evolve Martial Arts & Fitness athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and Covid-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Evolve Martial Arts & Fitness their instructors, officials, agents, and/or employees, other participants, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
MEMBERSHIP CANCELLATION -
If, for any reason, you need to stop training, either permanently or temporarily, you understand that it is your responsibility to provide written notice to Evolve Martial Arts & Fitness at least 30 days prior to cancellation or suspension. You understand that if you choose to come back to Evolve Martial Arts and Fitness, you will no longer be grandfathered into the current price you are in.
Failure to use the membership and utilize the programs and facilities at the premises does not relieve Member's liability for payment, regardless of circumstances.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
